Wound Care

At Symphony of Crestwood a holistic plan of care is designed for our wound patients. Our staff is trained to anticipate and care for their needs.

A nurse coordinator, assigned to each patient, works closely with family and physician to monitor progress and serve as a resource to address concerns. They collaborate with the discharge physician’s hospital to ensure appropriate follow up care is coordinated upon discharge from Symphony of Crestwood.

Our interdisciplinary team, Physician Specialist, Wound Ostomy Nurse, Dietitian, Social Worker, Physical Therapist, Occupational Therapist, Speech Therapist and nursing staff assess and reassess the plan of care. This has reduced our average recovery time and stay by 2 to 8 weeks.
